Expanded Services Creates Home Health Care One-Stop Shop

The additions of a pharmacy and a department of mental health program have put Phoenix Home Care and Hospice in a unique position in the realm of home health care. “We really have the full continuum of care,” says Brandi Crane, public relations manager. “We’re a one stop shop for home health care. That’s incredibly rare in home care.”

Under the umbrella of home health care Phoenix offers skilled nursing and physical, occupational and speech therapy, home health aides, IV infusion/therapy, in-home safety evaluations, and many other standard home health services.

It’s the additional services that have turned Phoenix into a one-stop shop for home health care.

Additional services include:

  • Mental health care
  • Private pay services, including nursing, nurse-aide or companion level assistance with activities of daily living (meal preparation, personal care, etc.) from a few hours a day to around-the-clock care
  • Medicaid services, including household chores, errands and shopping
  • Private duty nursing for children and young adults with specialized healthcare needs
  • Consumer directed services
  • Comprehensive disease management
  • End-of-life/hospice care
  • Pharmacy, including WellPack medication management

Phoenix Department of Mental Health (DMH) services were created to improve the lives of those with individuals with developmental disabilities including intellectual disabilities, cerebral palsy and more. The services are specifically designed so that clients may maintain self-sufficiency and remain in their own homes.

The pharmacy, including WellPack services, are available to anyone, not just Phoenix Home Care clients. For people who use the WellPack prescription delivery service, routine medications are sorted and packaged according to the time of day they are prescribed. Each triple-checked packet reflects the date, day of the week, the time of day the medications are to be taken and a list of the pills inside the packet.

Co-pays are the same as at other pharmacies and there are no delivery fees or hidden extra charges. “It’s nice not to have to make several trips to the pharmacy,” says Crane. “That and having all your medications organized for the month saves a lot of time and confusion for so many people.” Phoenix will manage client refills and communicate with patients and their doctors to ensure treatment is on the right track.
